Forms for Grant Holders
Important tasks for grant applicants
All Dalhousie Pharmacy Endowment Fund (DPEF) grant holders must complete either an [DOC - 46 kB] or a [DOC - 44 kB].
It is extremely important that either an interim or final report be submitted to the executive director of the DPEF by October 15th of each year.
The reports will be reviewed by the DPEF advisory board at their next meeting.
Please note: If a grant holder does not submit the required forms by the October 15 deadline, they will no longer be considered to receive further funds from the DFEF.
